  • Founded in 1986. Created as a ‘stand alone’ case management agency to assist adults in transitioning back into the community from long-term psychiatric hospitalization.

  • Senior citizens and persons with mental illness are offered both office and home-based services.

Service Overview…
  • Individuals can enter services through different “doors”.  Individuals can walk in to Pathways if they are in crisis.   They can receive services in their home, in the community or our facility.  They get help with daily living skills and receive ongoing services to help them live independently.

  •  Large psychiatric staff.

  • Central Pharmacy and Patient Assistance Programs.

  •  Residential Treatment – North Coast House.

  • Crisis intervention and emergency mental health services.

  • Lake County Crisis Hotline 24/7 (more than 18,000 calls in 2007).

  • Case Management (CPST – Community Psychiatric Support Treatment Workers) provide one on one support and education for independent living.

  • Senior Care - In-home support for seniors.
